United Administrative Services (“UAS”), the administrative office of the Healthcare Trust Fund, is the only authorized source of information concerning the administration of the Trust and the interpretation of the Plan provisions affecting the rights and duties of any Employee, retiree, or other person. All other sources, including without limitation, any Individual Trustee or officers and representatives of the Local Union, individual Employer or Employer Association (whether a Trustee or not) are completely unauthorized, and statements and opinions from them are not to be relied upon. Employees, retirees and other persons desiring information about the administration of the Trust, or a ruling as to their particular rights and duties under the Plans of the Trust, must request the same in writing from the Trust Fund Office.
